10 years ago to provide people with health servicesand programs through their family doctors.
Today, PCNs work with teams of other healthprofessionals, as well as offer programs such asPrescription to Get Active and Find-a-Doctor.

Married for 43 years, Bill and ColleenMontgomery have different approaches tolife. He has a quiet, cerebral nature; she hasan engaging and spirited persona. They do,however, share a dogged determination tosolve one of their biggest concerns—Bill’sincreasingly complicated and debilitatinghealth issues.
A dedicated and multidisciplinary teamof healthcare providers at the GeriatricAssessment and Support Clinic at the CalgaryWest Central Primary Care Network (PCN)recently joined the couple.
“It has been an amazing journey,” says Colleen,of their experience with the clinic, whichhas existed since 2009 and sees about 240new patients each year. “We have been veryimpressed.”Bill’s health problems started 30 years agowhen he had a cancerous tumour removedfrom his brain. Soon after, he experiencedminor mobility issues, memory blips and milddepression, but he continued to work as amechanical engineer in Calgary’s oilpatch.
Fast forward to June 2016: Bill was retired andhis symptoms were much worse. Plus he had
